Summary

This grant program is designed to support nonprofit organizations (or public entities) working to improve health and well-being in a defined geographic region. The foundation funds groups that are 501(c)(3) nonprofits, organizations operating under a fiscal sponsor, or tax-supported bodies like schools or local government institutions. For‑profit companies and individuals are not eligible for funding.

Grant requests can span a wide range of formats — general operating support, programs or projects, capital investments, and capacity‑building. There is particular emphasis on serving under‑resourced populations and targeting chronic or systemic health challenges through a “population health” lens. Its focus areas include things like healthcare access (physical, mental, oral, substance‑use), suicide prevention, trauma recovery, food security, physical activity, and affordable housing.

There are two main cycles for grant applications each year. One cycle is in the winter/spring and typically supports requests tied to healthcare access, suicide prevention, and trauma healing. The other is in summer/fall and prioritizes work around food, active living, or transitional housing. Multiyear grants are possible, but generally only for new or innovative projects (not for ongoing operational costs or capital). Smaller capacity‑building grants are also available in a more flexible, rolling way. These “mini-grants” are intended for nonprofits to invest in their internal development — for example through strategic planning, evaluation, technology upgrades, or leadership work — rather than to cover regular program or facility costs.

Grant awards vary: there’s no fixed minimum or maximum award, and the foundation tends to avoid being the sole or majority funder of any one project. Funding decisions are guided by how well a proposal aligns with the foundation’s mission, the potential impact in high-need communities, and how the applicant plans to measure success.

Organizations that are deeply embedded in community health, equity, and long-term systems change are most likely to benefit.
